Family Health Referral Form

Public Health Nurses (PHNs) from Carver County Public Health can work with pregnant women and families with a new baby in their home and at other community settings. PHNs partner with families, their healthcare providers and local community agencies to promote healthy pregnancies and birth outcomes, encourage healthy growth and development, and more.

If you would like to refer yourself or someone else for a visit with a PHN, please complete Public Health's Family Health Referral Form.
