The Community Health Improvement Plan (CHIP) is a strategic planning tool for improving the health of Carver County. The Carver County Public Health Department facilitates a planning process with a variety of partners, including non-profit organizations, public agencies, health care, businesses, and the faith community. The strengths, needs, and interests of the community drive the planning process. This information is then used to develop the CHIP, which includes specific strategies to enhance the health of the county.
