MnCHOICES is a single, comprehensive assessment, and support planning web-based application for long-term services and supports in Minnesota. It has replaced assessment tools that were used for developmental disability screening, long-term care, consultation, and personal care assistance assessment.  A Minnesota Department of Human Services initiative, MnCHOICES provides an assessment process for people of all ages, abilities, and financial statuses.  It can be used to determine eligibility for publicly fund programs and services.
