The Home and Community Based Care Department is responsible for case management services and adult protection investigations.  Programs are designed to support elderly and people with disabilities in their efforts to remain living as independently in the community as possible.  There are several different programs that are targeted to specific age groups and specific types of disabilities.

The staff uses the Minnesota Department of Human Services (DHS) MnCHOICES assessment and support application system to determine eligibility for a wide variety of programs and services for individuals of all ages.

The Home and Community Based Care Department adult protection unit is responsible for investigation of maltreatment of vulnerable adult reports.
