Adult Foster Care is a placement alternative for adults unable to live alone, but who do not need an institutional setting. Adult Foster Care provides a family setting for individuals who can live in the community.

Who Needs Care?

Adults with who are elderly, have developmental disabilities, or have a mental illness live in Adult Foster Care homes. People may range in age from 18 to 102. Many of these individuals attends school, day programs, or hold jobs.

How Long Is Care Needed?

Some adults need long-term care and could be members of your family for many years. Other adults need short-term care until they complete high school and/or learn the skills they need to live independently. Elderly persons may live in Adult Foster Care homes until their health requires total nursing care.

What Responsibilities Do Adult Foster Care Providers Have?

Adult Foster Care providers share a positive family atmosphere and the care, supervision, and understanding the adult needs. They provide meals, facilities for laundry, a private room (unless the adult agrees to share), guidance and assistance with money management, medications, transportation, community access, and other services as appropriate for the individual.

Reasons For Placement:

To provide a transitional step between independent living and institutional care within the community. The common goal for all adults living in Adult Foster Care homes is their desire to remain in the community and live purposeful lives.

What is the difference between Family Adult Foster Care (AFC & Community Residential Setting (CRS)?

An AFC home is one in which the care provider lives in the home that is licensed, and they are also the license holder. Individuals that need support and are receiving services that live in the AFC home become integrated into the life of that family. A CRS is a corporate licensed home in which there are staff that rotate in and out of the home to provide the necessary care for residents. The license holder for a CRS is typically an agency that manages multiple homes in a region. Both types of homes can be licensed up to 4 individuals, based on the bedroom and space accommodations in the living environment.

What is the difference between AFC with/without a 245D license?

An AFC with a 245D license is able to provide services and receive payments for individuals that receive waivered services (Developmental Disability Waiver, Brain Injury Waiver, Elderly Waiver, Community Alternative Care Waiver). An AFC without a 245D license is able to provide services through only the Elderly Waiver, and/or through private pay.

The licensing requirements do vary, based on the fact that when a 245D license is held, the Department of Human Services (DHS) will also conduct a separate licensing review in addition to the County licensing reviews that are completed.

How does an AFC home obtain a 245D license?

You must apply for a 245D license through the Department of Human Services (DHS). You have the option to obtain one tied to your own AFC license, or you could consider contracting with an agency who already holds the license, in which you would work under their umbrella. How much does it cost to get an AFC license?

There is a $500 Adult Foster Care licensing fee that will be made payable to Carver County HHS. A Fire Inspection will be required, which costs $50. There will also be miscellaneous expenses required to bring the home up to safety code; including but not limited to a proper fire extinguisher, adequate smoke detectors, weather emergency supplies, a guard for a fire place, and ample first aid supplies.
