Carver County Public Health provides outreach for the preventive healthcare program Child and Teen Checkups. Child and Teen Checkups includes well-child medical exams and dental care, at no cost to eligible children and teens. These checkups include specific screening components to improve the health of children and teens, and limit long-term outcomes of undetected health problems.
Eligibility Requirements
- Children and teens, birth through age 20 years
- Enrolled in Medical Assistance (MA)
Where are Child and Teen Checkups offered?
A Child and Teen Checkups exam can be provided by your regular healthcare provider.
What does a Child and Teen Checkups exam include?
- Complete physical exam
- Developmental screening
- Hearing and vision checks
- Lab tests, including lead testing
- Vaccinations
- Information about good physical health and nutrition
- Information about thoughts, feelings, and relationships with others
- Referral to the dentist
- Referrals to other health specialists, as needed
- Form completion for Women, Infants and Children (WIC), Head Start, school, sports, or child care
When should Child and Teen Checkups exam be scheduled?
- Between birth and 1 month of age
- At 2, 4, 6, 9, 12, 15, 18, 24 and 30 months of age
- Every year from age 3 through age 20
Child and Teen Checkups can be used for medical checkups required for Head Start, WIC, school, or child care.
